Under the guidance and supervision of the Director of Youth Services Division or designee, the HIV Community Health Worker will be responsible for coordinating and implementing the Outreach Activities for the HIV/AIDS Program in the service area. The Outreach Worker will conduct outreach activities to targeted areas of identify persons at women from communities of color at risk for HIV/AIDS and other STDs and help to link them to comprehensive health services. Responsible to provide HIV/AIDS education and classes to increase the awareness, knowledge and promote healthy lifestyle behaviors as it relates to HIV/AIDS.

Essential Functions:

  • Follow up on case finding attempts via phone or home visits.
  • Administer pre/post testing and comfortable with administrating HIV/HepC/STD testing, per guidelines.
  • Coordinate and co-facilitate HIV/AIDS support group meetings.
  • Coordinate outreach activities with community-based organizations, businesses, and schools.
  • Design peer-teen outreach initiatives.
  • Conduct education sessions on the risk factors, intervention strategies and community resources.
  • Link participants to community services for primary care services as assigned by Trillium Health.
  • Develop strong working relationships w/businesses and organizations in our service area.
  • Represent Ibero in external meetings and forums.
  • Work with external coalitions to plan new outreach initiatives.
  • Make referrals to community agencies as appropriate.
  • Look for new educational programming opportunities/places within communities of color specifically Latina population
  • Provide support and facilitate the coordination of community events, health fairs, and other related health education activities to support programs, particularly tailored to the needs of the community.
  • Maintain data base of patient encounters and outreach data.
  • Complete and submits all required monthly progress/program reports.
  • Maintain educational materials.
  • Works outside regularly scheduled work hours as needed including evenings and weekends.
  • Supports program planning efforts with the Women's Prevention Program.
  • Attend monthly CHI Department Meeting and WPP meetings
  • Any other duties as may be assigned by the immediate supervisor.

 Skills and Abilities:

  • High School Diploma, College Degree Preferred.
  • At least three years of experience or any combination of training and experience which would provide the required knowledge and abilities to perform job duties.
  • Bilingual English/Spanish speaking.  
  • Possesses at least base knowledge of HIV/AIDS, HepC and PrEP knowledge a must.
  • Capacity to conduct street and community outreach targeting at high-risk populations.
  • Experience working with diverse populations.
  • Must maintain confidentiality regarding agency and constituent information.
  • Valid N.Y.S. driver's license and able to provide own/client transportation.
  • A combination of training and experience other than the specified, if judged to be adequate for the job, may be considered.

Physical Demands:

The position does require occasional standing, squatting, lifting of up to approximately 10 lbs. And frequent sitting.
