Summary:

Under the supervision of the Day Habilitation program Coordinator, the Day Habilitation Team Leader is responsible for ensuring delivery of quality services to all enrolled individuals. The Day Habilitation Team Leader is responsible to ensure all DSPs complete all assigned daily tasks, routines and reports any concerns to the Program Manager. The Day Habilitation Team Leader will be part of an Interdisciplinary Treatment Team. This is a 40 hours per week position.

Day Habilitation Program Team Leader:

  • Under the supervision of the Day habilitation Program Coordinator, the Day Habilitation Team Leader will foster independence in the individuals through Day Habilitation Action Plans.
  • Complete daily communication log assignment sheets by assigning individuals and responsibilities to DSPs each morning.
  • Ensures all assigned tasks such attendance, appointments and outings are completed.
  • Ensures all billing and outcomes sheets are completed in a timely manner for each individual.
  • Meets weekly and as needed with the Day Hab Program Coordinator to report any issues or concerns, review the past week and plan for the following week. Reviews all progress notes done in the system (ECR) on a daily basis as well as to ensure monthly summaries are completed and up to date. This check-in will take place on a weekly basis.
  • Reviews and signs off on all outing forms.
  • Attends all life plan meetings with program coordinator to ensure updates are accurate and according to the changes desired by individual and staff.
  • Ensures all volunteer activities are completed weekly.
  • Contacts IRA managers, family members, and care managers as needed during the day.
  • Communicates and coordinates with the nursing staff to ensure individual's medical needs are met.
  • Prepares documentation, assigns and ensures monthly summaries are completed by the 15th of each month.
  • Responsible to oversee the cleanliness and maintenance of the program facility on a daily basis, and to assign staff cleaning responsibilities.
  • Responsible to submit work orders for repairs as needed, in conjunction with the Program Coordinator.
  • Maintains professional dress and attitude as a role model for individual's and staff.
  • Ensures transportation for individuals to and from program, as well as to various activities, including volunteer activities, and for other purposes occurs as scheduled.
  • Is responsible to maintain the agency vehicles to ensure safety. Submits all gas receipts to Program Coordinator on a weekly basis.
  • Ensures that fire safety needs are met, including planning and executing quarterly fire drills or as needed in conjunction with the Program Coordinator.
  • Maintains individual's records on the OPWDD Choices systems as needed, as well as our electronic record ECR.
  • Ensures the safety and wellbeing of all individuals at all times.
  • Attends mandatory training and/or meetings as scheduled, including those held outside of regular work hours.
  • Maintains up to date all necessary first aid and CPR certifications.
  • All other duties as assigned by the supervisor.

Authority:

This position exercises considerable initiative and independence in setting goals, accomplishing activities, and completing consumer outcomes. Under the general supervision of the Program Coordinator, the Day Habilitation Team Leader will maintain a professional and responsible work environment for the program.

Skills and Abilities:

  • High School diploma or GED
  • Current NY State driver's license
  • Experience working with community human service providers and working with Developmentally Disabled population
  • Must be bilingual
  • Basic cooking skills
  • Ability to follow oral and written directions.
  • Ability to maintain confidentiality and to follow all Ibero Policy and Procedure.
  • A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO

Physical Demands:

The position does require occasional standing, squatting, lifting of up to approximately 10 lbs.
