Under the supervision of the Coordinator of HCBS, the HCBS Direct Support Professional is responsible for ensuring delivery of quality services to all individuals in the HCBS programs.

  • Reports to Coordinator of Home & Community Based Services.
  • Engages individuals in a manner appropriate to their skill level.
  • Monitor and record consumers progress to ensure that goals and objectives are met.
  • Completes Residential Habilitation notes monthly according to standard.
  • Writes monthly summaries for everyone served, maintains appropriate and up to date documentation for each client.
  • Guides individuals with daily activities.
  • Accommodates the clients family and the individual's needs in the home and in the community.
  • Provide HCBS services directly to all assigned individuals.
  • All other duties as assigned by the supervisor.


This position exercises considerable initiative and independence in setting goals, activities and individual reviews. Under the general supervision of the HCBS Coordinator, the HCBS DSP will maintain a professional and responsible work environment for the program.

Skills and Abilities:

  • High School diploma or GED.
  • Experience working with the developmentally disabled preferred.
  • Six months Human Service experience
  • Must be bilingual (English/Spanish).
  • Valid NYS driver's license and reliable transportation.
  • A combination of training and experience other than the specified, if judged to be adequate for the job, may be accepted by the President and CEO.

Physical Demands:

The position does require occasional standing, squatting, lifting of up to approximately 10 lbs.
