Summary:

Under the supervision of the Senior Residential Manager, the Residential Coordinator will provide supervision and guidance to the department, personnel and resources assigned to him/her.  The Residential Program Coordinator ensures that residential services are delivered in a fashion that meets Medicaid Standard, OPWDD and Ibero policies and procedures. 

Essential Functions:

  • Reports directly to the Senior Residential Manager
  • Residential Coordinators will ensure that the Residential Habilitation Plan is consistent with information in the individuals Life Plan (LP), the Behavior Support Plan (BSP) and Medical consultation forms and Sleep charts
  • Update the Individual Protective Oversight Plan (IPOP) as needed and no less that 2X per year (every 6 months and after annual LP review
  • Review and reconcile Personal Allowance finances on a weekly basis
  • Complete monthly summary for all Res Hab. Notes. Specifically indicating the resident response to services and the level of prompting.
  • Supervise and conduct fire drills according to provided fire drill schedule
  • Ensure the Fire Evacuation Plan is reviewed monthly to reflect the needs of the home identified during fire drills
  • Meet with Medical Liaison once per week to review any changes in the Medication Administration Record (MAR)
  • Review Daily Res Hab. notes to ensure staffs are completing notes as per Medicaid billing standards
  • Send any updates and information to MSC so they can amend the LP and notify advocates and relatives
  • Maintains up to date all mandatory trainings for staff as well as self including but not limited to SCIP Medication Certification, Van, Fire, OPWDD etc.
  • Adhere to all Ibero policies and procedures
  • Attend mandatory education and training modules as scheduled and maintains required certifications
  • Perform all other duties relevant to the position or as assigned by supervisor

Consumer Protection Duties:

         Follows Ibero incident management policy and procedure

         Manages the group home in a fashion which minimizes potential for abuse, mistreatment /negligence in the care and treatment of consumers who live in the group home

         Manages in a fashion that protects the rights of the residential consumers

         Ensures that all staff are trained in, understand and follow Ibero incident management policy on an ongoing basis

         Takes appropriate follow-up action after an incident has occurred

  • Manages and oversees Personal Allowance needs

         Assists with completing internal audits

Fire Evacuation Duties:

         Ensures that fire drills are conducted on a timely fashion as required by agency Policy and Procedure. 12xs per year as per schedule.

         Maintains current fire evacuation plans as required by Ibero and OPWDD for all individual consumers.

         Ensures that immediate corrective action plans are developed and implemented related to any evacuation deficiency identified in the course of a fire drill or identified in an internal or external audit. Supervise and witness 6 drills per year.

         Provides timely and complete training regarding fire evacuation duties to all direct care staff who are assigned to the group home.

         Maintains the Fire Book in a fashion that ensures that it is complete and current per Ibero policy requirements.

         Maintains current and complete documentation of all fire evacuation activities as required by Ibero Policy and OPWDD Regulations.

         Maintains fire extinguishers and home monthly smoke alarm inspections on schedule..

Residential Habilitation Duties:

         Manages in a fashion which ensures that Residential Habilitation Services Plans are developed on a timely fashion for each individual who resides in the group home, and ensures Medicaid billing standards.

         Ensures through management oversight and direct supervision that direct care staff are assigned and deliver Residential Habilitation Services in a fashion which follows the Individual Residential Habilitation Plan for each Consumer.

         Ensures that each assigned Direct Support Professional receives training in Residential Habilitation that is required in order to ensure Direct Care staff performance which meets Ibero and OPWDD requirements for Residential Habilitation development, delivery and documentation as per Medicaid standards.

         Monitors daily documentation of provision of Residential Habilitation Services and takes follow-up action as necessary to ensure compliance with Ibero and OPWDD documentation requirements.

         Ensures that documentation required to support billing for Residential Habilitation is submitted as required.

         Demonstrates awareness of the meaning and importance of the principles of Inclusion, Independence, Individualization and Productivity and implements these principles in day to day management of program delivery in the home.

Clinical Services Integration:

         Effectively communicates need(s) for clinical supports in a timely and appropriate fashion to professional services providers and arranges for provision of professional consultation as necessary.

         Monitors to ensure that any necessary follow-up to professional clinical consultation is identified and communicated to Direct Support staff in a clear and concise manner.

         Ensures that Direct Support Staff receive training necessary to implement follow-through of clinical supports within the home as determined by the involved clinical professionals.

         Ensures that staff is assigned on each shift to deliver in-home extension of clinical services as planned by clinical professionals.

         Supervises Direct Support Staff and monitors activities in accordance with Ibero policy and in a manner sufficient to ensure that Direct Support Staff properly document delivery of clinical services plans on a daily and shift by shift basis.

Staffing and Budgeting Duties:

         Create monthly staff schedule that ensures sufficient staff are on-duty to meet minimum staffing (staff to individual ratio) requirements and to carry out planned programmatic requirements

  • Ensures that all staff are recruited and hired on a timely manner, following Ibero Selection Procedures
  • Works as a DSP to alleviate overtime when needed

         Manages spending within allocated levels in Personal Services, Overtime and Non-Personal Services Categories

         Implements Internal Control policy and procedures related to staff time and attendance rules, spending, personal allowance management, equipment and vehicles as defined by Ibero policy and procedure.

  • Generates work orders through maintenance department on weekly basis.

Property Management:

         Ensures that staff is assigned and necessary resources are expended to maintain a clean, safe and sanitary living environment within the home and surrounding grounds.

         Maintains an attractive and comfortable living environment within the home and surrounding landscape.

Staff Supervision:

         Maintains overall supervision and direction of staff assigned to the home.

         Implements appropriate corrective action with staff as required.

         Ensures that all staff within the home receives required training.

         Completes staff performance evaluations on a timely manner and in a fashion that recognizes superior performance and identifies substandard performance together with directions related to improvement of any sub-standard performance.

Authority:

This position holds a relatively high degree of independence within the framework of the agency's policies and procedures. The individual must be resourceful and able to display initiative and understanding of management systems and interpersonal relations.

Skills and Abilities:

  • Bachelor's degree in human services or related field with at least 12 months' work experience in the Developmental Disabilities field, or an equivalent combination of education and experience will be considered. High school diploma/equivalency required.
  • Two years' experience with first line supervision of staff preferred.
  • Must possess a valid NYS driver's license and meet Ibero Vehicle Operator Requirements.
  • Bilingual Spanish and English preferred but not required.

Physical Demands:

The position does require occasional standing, squatting, lifting of up to approximately 50 lbs. And frequent sitting.
